Is hyperlactatemia a useful screening marker of severe sepsis in hemato-oncologic malignant patients?

چکیده
Introduction Hyperlactatemia is often associated with poor clinical outcomes. Mortality is increased when lactic acidosis with low-flow states or sepsis. For improving poor prognosis, early detection and intervention of hyperlactatemia are important. There are call for monitoring blood lactate level. However, hyperlactatemia is often associated with type II lactic acidosis in hemato-oncologic malignant patients.

Shock Index and Early Recognition of Sepsis in the Emergency Department: Pilot Study
INTRODUCTION Screening for severe sepsis in adult emergency department (ED) patients may involve potential delays while waiting for laboratory testing, leading to postponed identification or over-utilization of resources. The systemic inflammatory response syndrome (SIRS) criteria are inaccurate at predicting clinical outcomes in sepsis. Shock index (SI), defined as heart rate / systolic blood ...
متن کاملReclassifying the spectrum of septic patients using lactate: severe sepsis, cryptic shock, vasoplegic shock and dysoxic shock
OBJECTIVE The current definition of severe sepsis and septic shock includes a heterogeneous profile of patients. Although the prognostic value of hyperlactatemia is well established, hyperlactatemia is observed in patients with and without shock. The present study aimed to compare the prognosis of septic patients by stratifying them according to two factors: hyperlactatemia and persistent hypot...
متن کاملPersistent sepsis-induced hypotension without hyperlactatemia: is it really septic shock?
PURPOSE The prognostic value of hyperlactatemia in septic shock is unquestionable. However, as current definitions do not include hyperlactatemia as a mandatory criterion, some hypotensive patients may be diagnosed as having septic shock despite exhibiting normolactatemia. The significance of persistent sepsis-induced hypotension without hyperlactatemia is unclear. Is it really septic shock? Ou...

Lactate was first described by Berzelius in 1807 when he discovered it in its modified form in meat juices. It has been used as a marker of cellular hypoxia and tissue malperfusion, and hyperlactatemia has been associated with postoperative complications and mortality. Notably, high blood lactate concentrations have been associated with increased mortality and morbidity in children after cardia...
متن کاملبررسی سیتوکینهایTh1-Th2 در سطح سلولی در بیماران مبتلا به سپسیس و سپسیس شدید
Background: Sepsis is the leading cause of death in critically ill patients throughout the world. The incidence is increasing despite the major advances in the development of antimicrobial agents and other supportive treatments. Based on multiple studies, it has been shown that patient outcome depends on Th1 and Th2 cytokine response. Moreover, whenever the Th2 response is predominant, the seps...
